Documents with payments enclosed must be mailed to WVBORC, P O Box 40329, Charleston, WV 25364.

Documents without payments enclosed may be mailed to WVBORC, 106 Dee Drive, Suite 1, Charleston, WV 25311.

NOTICE FOR WALK-IN APPLICANTS: DUE TO NEW PROCESSING PROCEDURES, YOU MAY BRING YOUR APPICATION TO THE OFFICE BUT, WE ARE NO LONGER ISSUING A "SAME DAY" LICENSE.

EMPLOYERS: IF YOU NEED TO VERIFY IF A PRACTITIONER HAS A VALID LICENSE PLEASE CLICK "VERIFICATIONS" AND YOU MAY VIEW OR PRINT A COPY FOR YOUR RECORDS. THESE VERIFICATIONS MEET THE REQUIREMENTS MANDATED BY JACHO.
